Mighty Oak supports individuals with disabilities and their families at every stage of the Self-Determination Program (SDP).
Whether you’re just learning about Self-Determination Program or already participating and need ongoing support, we meet you where you are. We provide guidance and advice so you can make informed choices with confidence.
Independent Facilitation service comes at no direct cost to you—services are funded by the Regional Center or included in your SDP spending plan.

Getting Started with SDP
Support begins with person-centered planning. Through one or more facilitated meetings, goals, priorities, and support needs are clarified and documented. The outcome is a comprehensive Person-Centered Plan that reflects the individual’s voice and provides a strong foundation for SDP discussions with the Regional Center.

Transition into SDP
Once a Person-Centered Plan has been shared with the Regional Center, support continues through the SDP transition process. This includes guidance to ensure required steps are completed, such as finalizing an authorized budget, developing an approved spending plan, and establishing a Financial Management Services (FMS) provider.

Ongoing SDP Support
After enrollment in SDP, ongoing support is available to help navigate changes and requirements over time. This may include assistance with spending plan updates, annual renewals, and review of expense reports to support continued alignment with program guidelines and individual goals.

Our Experience
Sophia brings over 20 years of experience in communications, marketing, and client advocacy at top global agencies and a Fortune 50 technology company. Driven by her family’s own experience navigating complex systems, she now supports individuals with developmental disabilities and their families as a Certified Independent Facilitator.
She provides person-centered planning and independent facilitation services, helping clients clarify goals, understand their rights, explore options, and navigate services to build lives on their own terms. Sophia is an active member of The IF Collective Community, staying current on policy and best practices. She is fluent in English and Chinese.
